AN occupational therapist from Barry has been struck off for falsifying service user records and poor record keeping and causing delay for service users, whilst working in the social services team at Vale of Glamorgan Council.
A Health & Care Professions Council disciplinary panel found that Eryl Owen Ring failed to maintain adequate and contemporaneous records of her activities with service users. It was told that some entries were made as long as seven months after the contact.
The panel was also told that Ring failed to follow up on an assessment of one service user for nine months.
And, when the person's mother complained she then attempted to show she had done some follow-up work by falsifying two emails to colleague in the housing team dated several months after he had left the team.
Striking off Ring, who had already been dismissed from her job disciplinary panel chair, Raymond Pattison said: "The failure to follow up an intervention for nine months represents a serious departure from accepted practices and resulted in significant delay for a vulnerable service user.
"The Panel was of the view that these were not isolated lapses, they spanned a number of years, were repeated and the dishonest actions were also repeated.
"There is a very high risk of recurrence as the registrant has shown no insight and there is no evidence of any remedial action."
He said the Panel took the view that the most appropriate action was to strike Ring from the HCPC register.
Eryl Owen Ring was not present or represented at the three day hearing in Cardiff.
